To better assist our patients and their families with the transition from hospital to home, or to other facilities, SRMC’s care staff includes a full-time Social Services Coordinator.

Prior to discharge, our Coordinator will work with families to develop a discharge plan, and if necessary, assist in identifying alternative placements for the patient such as assisted living or a nursing home.

The Coordinator can also provide referrals to local outside agencies such as Home Health, Department of Aging, Futures Unlimited, SRS, or an area hospice, depending on the patient’s and family’s needs.
